What doses does the Wegovy pill come in?
The Wegovy pill is available in 4 strengths: 1.5mg, 4mg, 9mg, and 25mg. You take the pill once daily, starting at the lowest 1.5mg dose and gradually increasing once a month until you reach the maximum 25mg dose, also known as the maintenance dose.
What is the starting dose of the Wegovy pill?
The starting dose of the Wegovy pill is 1.5mg per day. You will take this dose daily for the first month (30 days) of your treatment.
The purpose of this dose is to gradually get your body used to semaglutide. You may not notice many changes to your appetite or weight at this dose, but that is normal. However, you may experience side effects, such as feeling sick (nausea), which are often mild and will improve as your body adjusts to the medication.
Wegovy pill 4mg dose
After 1 month on the starting dose, you will move up to the 4mg Wegovy pill dose. You will take this once daily for a month. You may start to notice that you feel fuller sooner and for longer after eating, are snacking less between meals, and that you are naturally eating smaller portions at meal times.
You may also notice gastrointestinal side effects becoming more pronounced. This is usually a result of the increased dose and will usually settle as your body gets used to it.
Wegovy pill 9mg dose
From month 3, your Wegovy pill dose will increase to 9mg. The effects of the Wegovy pill may become stronger over the next month, so you may feel full more quickly or feel less hungry between meals. It also means you may temporarily experience side effects again as your body gets used to the increased dose.
What is the highest dose of the Wegovy pill?
The highest dose of the Wegovy pill is 25mg per day. This is also the maintenance dose at which the most significant weight loss, 16.6%, occurs over 64 weeks.
If you follow the Wegovy pill dosage schedule exactly, it will take 3 full months to reach this dose, but it may take you longer if you remain on a lower dose for more than a month.
Appetite suppression at this dose is at its strongest, and you may notice the return of side effects when you increase to the 25mg dose. However, these should improve as your body adjusts to the increased dose.
What is the maintenance dose of the Wegovy pill?
The 25mg dose is the maintenance dose of the Wegovy pill. Unlike the Wegovy injection, the Wegovy pill only has 1 licensed maintenance dose. Once you reach the 25mg daily dose of the Wegovy pill, this is the strength you will stay on for long-term weight management.
What is the Wegovy pill dosing schedule?
The Wegovy pill dosage schedule starts on the lowest dose and gradually increases until you reach the maximum maintenance dose as follows:
- Month 1 – 1.5mg once daily
- Month 2 – 4mg once daily
- Month 3 – 9mg once daily
- Month 4 onwards – 25mg once daily
You will usually stay on your current dose for a month before titrating up (increasing) to the next dose. You’ll do this every month until you reach the maintenance dose at month 4. This is the dose you will remain on for long-term treatment and is the strength at which the most significant weight loss occurs.
What if I can’t tolerate the 25mg dose?
If you cannot tolerate the 25mg dose of the Wegovy pill, it's important to discuss this with your doctor because you may not need to stop treatment. They may suggest either remaining on a lower dose or switching to the Wegovy injection.
Can I stay on a low dose of the Wegovy pill?
Yes, it is possible to stay on a lower dose of the Wegovy pill if you are finding that the side effects of your current dosage are difficult to manage. Moving to a lower dosage can help reduce the likelihood and severity of side effects, as there is less semaglutide in your body.
Some people find that sticking to a lower dose for longer than the month specified in the Wegovy pill dosage schedule gives their body more time to adjust to the medication. This means that you may be able to increase your dose later with a lower risk of side effects.
If you remain on a lower dose for the duration of your treatment, this is likely to result in lower weight loss results. That’s because the average weight loss of 16.6% on the Wegovy pill in clinical trials was achieved on the highest 25mg dose compared to 2.7% with a placebo.
Can I switch to the Wegovy injection?
If the Wegovy pill is not right for you, then your doctor may advise you to switch to the Wegovy injection. Because the injection delivers semaglutide under your skin and into your bloodstream, it bypasses the digestive system and avoids the irritation that the Wegovy pill’s coating may cause. While the injection can cause similar side effects to the Wegovy pill, gastrointestinal side effects such as nausea and vomiting are slightly less common with the injection.
Can I cut the Wegovy pill in half?
No, you should not cut the Wegovy pill in half to lower the dose or make it easier to swallow. You must swallow it whole because the Wegovy pill has a special coating that protects the active ingredients from stomach acid, allowing the medication to be absorbed into your bloodstream. If you split the tablet in half, the medication will be less effective.
If you think your current dose is too high, speak to your doctor, as you may be able to move to a lower dose.
What should I do if I miss a Wegovy pill dose?
If you miss a Wegovy pill dose, skip the missed dose and take your next dose at your usual time the following morning. You must not take a double dose to make up for a missed dose.
If you usually take your Wegovy pill in the morning as soon as you wake up, but you have woken up late (within about 1 hour), and you have not eaten, had a drink, or taken any other oral medications, you can take your usual dose. You will still need to wait 30 minutes before eating or drinking, as usual, so you may need to adjust your routine slightly to accommodate this.
If you keep forgetting to take the Wegovy pill and it has been a week or more since you last took it, speak to a doctor before restarting the medication. That’s because the dosage schedule is based on the pill being taken daily, so if there are any lengthy gaps in your treatment, a doctor may need to review your current dose.
Frequently missing your Wegovy dose can affect your overall weight loss progress. It must be taken consistently at the same time every day to be effective, as this ensures a steady level of semaglutide in your system, acting on your GLP-1 receptors to suppress your appetite. If you keep forgetting to take the Wegovy pill, you may experience:
- slow or reduced weight loss results
- more intense side effects
- slowed titration, as you may need to restart on a lower dose to help your body readjust to oral semaglutide
You may find it useful to set an alarm on your phone or to download a pill reminder app to prompt you to take the Wegovy pill. It can also help to keep the tablets somewhere you can see them and to take them at the same time every day so that taking them becomes a habit, just like brushing your teeth or showering.
Does the Wegovy pill dosage affect side effects?
Yes. Wegovy pill side effects are most likely to occur when you first start your treatment or when you increase your dose. This happens because your body is adjusting to a higher dose of semaglutide, but these effects usually improve or resolve within a few weeks.
Do side effects get worse at higher doses?
For some people, side effects may become more intense as their dose increases. At the lower Wegovy pill 1.5mg and 4mg doses, side effects tend to be milder because there is less semaglutide in your system. However, the Wegovy pill dosage schedule helps your body gradually adjust to semaglutide over several months, reducing the risk of side effects.
What should I do if I can't tolerate a dose increase?
If you are finding a Wegovy pill dose difficult to tolerate, you may not need to stop taking the medication. You should speak to your doctor because they may recommend one of the following options:
- staying on your current dose for longer to give your body more time to adjust
- temporarily reducing your dose
If your side effects are severe or you are experiencing persistent vomiting, severe stomach pain, dehydration, or you are concerned, speak to a doctor straight away for more advice.
What is the difference between the Wegovy pill and injection dosage?
Wegovy pills and injections contain the same active ingredient, semaglutide, but require very different doses because of how they are absorbed. The pill is taken daily in doses ranging from 1.5mg to 25mg, while the injection is taken weekly in doses ranging from 0.25mg to 7.2mg.
Why do the Wegovy pill doses look higher than the injection doses?
The Wegovy pill has a higher dosage than the injection because oral semaglutide is absorbed much less efficiently than injected semaglutide.
When you inject Wegovy, semaglutide is absorbed directly into the bloodstream. However, when you swallow the Wegovy pill, it has to travel through the digestive system before it can be absorbed. While the pill’s SNAC coating helps protect the medication from your stomach acid and improves absorption, only a small amount of each dose actually enters the bloodstream. So, the Wegovy pill doses are much higher to ensure that enough semaglutide can enter the bloodstream to be effective.
Wegovy pill vs Wegovy injection dosage
The differences between the Wegovy pill and injection doses are shown in the table.
| Wegovy pill | Wegovy injection | |
|---|---|---|
|
Month 1 |
1.5mg/day | 0.25mg/week |
| Month 2 | 4mg/day | 0.5mg/week |
| Month 3 | 9mg/day | 1mg/week |
| Month 4 | 25mg/day (maintenance dose) | 1.7mg/week (maintenance dose) |
| Month 5 | 25mg/day (maintenance dose) | 2.4mg/week (maintenance dose) |
| Month 6 | 25mg/day (maintenance dose) | 7.2mg/week (maintenance dose) |
The Wegovy pill only has 1 maintenance dose, which is the highest 25mg dose, compared to the Wegovy injection, which has 3 (1.7mg, 2.4mg, or 7.2mg).
Because Wegovy pill and injection doses are not interchangeable, you cannot simply switch from one to the other without medical guidance. If you are considering switching from the Wegovy injection to the pill, or vice versa, then speak to a doctor first who can advise you on which dose is suitable for you.
